In-Home Care Agency

Community Care Connections of Medicalodges is a Home and Community Based agency providing in-home services specializing in the care and needs of the frail elderly, and physically disabled private client. We offer quality home care and geriatric care management services in Southeast Kansas, serving over 13 counties. We assist individuals to obtain their highest level of independence, health, and well-being. We recognize and support each individual’s desire to remain self reliant and in their own home.

We provide services to private pay, self-directed and Medicaid approved clients, 24 hours a day, including all weekends and holidays. Service times can range from a few hours once a week to several hours each day. Services are tailored to the individual’s needs and desires.

Our staff consists of RN’s, LPN’s, Certified Medication Aides, Certified Nurse Aides, and highly trained Personal Care Attendants. The two levels of care we offer are:

Level I Services

Assistance with dressing, grooming, household cleaning, meal preparation, laundry, appointments, errands and shopping.

Level II Services

Assistance with bathing, mobility, vital signs, range of motion, transfer & lifting, catheter care, ostomy care, and medications supervision.

Wellness Monitoring

A registered nurse (RN) and/or LPN will make routine visits to the home to assess and monitor any treatment program prescribed by your physician or Case Manager. Education and counseling are available for your health care needs.

Community Care Connections delivers quality care that ensures confidence and trust in our agency and staff. We make every effort to provide for the protection, security, and confidentiality of our clients through routine background checks, drug testing and monthly in-service training programs. Quality assurance visits and phone calls are made routinely to assure that our clients and their families are completely satisfied with the services we provide.
